• The State of Wisconsin has an Immunization Law (State Statute 252.04) requiring students through grade 12 to receive a minimum amount of vaccines/doses.  The school nurse and other school desingated personel are responsible for monitoring immunization compliance.  All districts must be compliant with immunizations by the 40th day of school each year.  The district has the option to exclude students from the 30th day of school onward until compliant with vaccines.

    To be compliant with vaccines students need to have the minimum required vaccines/doses or a waiver on file with the school prior to the compliance deadline.  A waiver can be signed for personal, religious, or health reasons.  If signed for health reasons, a physician needs to sign off that a student is not eligible to receive the vaccine.  If your child is on an alteranate vaccine schedule, you may consider signing a waiver to be compliant with the state requirments as there can be penalties (fines or exclusion) for parents who's children are not complaint.  If you sign a waiver your child can still receive the waived vaccine at any time. 

    Kindergarten and 6th grade are milestone ages/grades where students have required immunizations due.  Don't wait until Fall to get your child vaccinated.  If you have questions about your child's vaccine record please contact me or call Portage County Health and Human Services 715-345-5350 and request to speak with a public health nurse.

    • MMR (Measles, Mumps, Rubella)
    • Varicella (Chicken Pox)
    • Hepatitis B
    • Tdap (Tetanus, Diptheria and Pertussis - 6th grade Booster)

    Highly Recommended (not required):

    • Meningitis
    • HPV
    • Influenza
