All students participating in a co-curricular athletic activity must have a physical or alternate year form filled out before the first day of practice. Parents and students also need to complete the Athletic Participation Form and show that they have read all of the information and completed all of the steps to participate. If students did not get this information from their coach, links to the information and forms are below and on the homepage of the website. If you have questions, please call the school office.
